    Financial Support

    Emergency Financial Assistance (EFA) emerges as a lifeline in times of crisis, providing critical support to individuals and families grappling with unforeseen financial challenges. This essential service aims to alleviate immediate economic hardships triggered by sudden events such as medical emergencies, natural disasters, or unexpected job loss. EFA programs are designed to bridge the gap between financial distress and stability, offering timely aid to cover essential needs like housing, utilities, medical expenses, and other pressing necessities. 

    Connect2uplift’s program provides short-term financial assistance with critical living expenses, such as:

    • Utilities and fuel assistance
    • Food
    • Medication
    • Medical/vision care
    • Transportation (PT1 submitted by PCP or other providers)
    • Rental assistance
    • Other emergency needs

    The EFA program helps you maintain stable health or recover from a serious health issue.

    Case Management Services Include:

    In the dynamic landscape of HIV/AIDS care and support, effective Case Management Services play a pivotal role in enhancing the quality of life for individuals living with HIV. These services form a comprehensive and client-centered approach to address the multifaceted challenges associated with HIV, aiming to optimize health outcomes, promote adherence to medical treatment, and foster holistic well-being. Case Management in the context of HIV/AIDS involves a collaborative and coordinated effort to assess, plan, implement, coordinate, monitor, and evaluate the various aspects of an individual’s care.

    • Education and information about HIV and transmission
    • Assistance with applying for Social Security, disability, Medicaid, and food stamps, housing, rental, and fuel assistance.
    • Referrals for medical and dental/oral care and treatment.
    • Referrals for legal assistance for advanced directives and wills
    • Emergency housing and utility financial assistance for eligible persons.
